Avaliação da efetividade, segurança e aceitabilidade de medicamentos antidepressivos em pacientes oncológicos: revisão de revisões sistemáticas

Introdução: A depressão é prevalente em pacientes com câncer, comprometendo a qualidade de vida e a adesão ao tratamento oncológico. O uso de antidepressivos é frequente nesse contexto, mas a evidência sobre sua efetividade, segurança e aceitabilidade permanece incerta. Objetivo: Avaliar a efetividade, segurança e aceitabilidade na utilização de antidepressivos em pacientes oncológicos durante o tratamento antineoplásico. Métodos: A pergunta norteadora para esta revisão foi: qual é a efetividade, segurança e aceitabilidade do tratamento farmacológico da depressão em pacientes adultos oncológicos? Foi realizada uma revisão de revisões sistemáticas (overview), com busca nas bases de PubMed e LILACS, incluindo publicações em português, inglês e espanhol. Foram selecionadas, por meio de dois pesquisadores e de maneira independente, revisões que investigaram antidepressivos em adultos diagnosticados com câncer e depressão. Resultados: Nove revisões sistemáticas foram incluídas, abrangendo ensaios clínicos randomizados publicados entre 1985 e 2019. A mianserina apresentou resultados consistentes de maior efetividade em comparação ao placebo. Fluoxetina, paroxetina, escitalopram e reboxetina também demonstraram benefício, embora com maior vulnerabilidade nos resultados. A segurança foi considerada aceitável para a maioria dos fármacos, exceto fluoxetina e paroxetina, associados a maior ocorrência de eventos adversos. A aceitabilidade variou conforme o medicamento, sendo mais favorável para a mianserina. Conclusão: Antidepressivos demonstram potencial no manejo da depressão em pacientes com câncer, destacando-se a mianserina e alguns ISRS. Contudo, a heterogeneidade e as limitações metodológicas dos estudos sugerem a necessidade de ensaios clínicos de maior qualidade para embasar recomendações clínicas robustas.
